Exception Request for Office Space
This Exception Request for Office Space form must have signatures by the department chairs or senior associate dean or unit, then sent via email to Facilities Space Planning Department. They will be reviewed by the space committee before being signed off by the Dean.

New Hire Form
Completion of this New Hire Form by the Hiring Supervisor/Manager with the proper signatures is required. Please download both forms New Hire Form and the Keys Authorization Form, then submit in Facilities & Planning ServiceLink: SOM - SOM - Portal (service-now.com).
Space Request Form
Completion of the Space Request Form, to start the space request process, including request for comps, offsite location, and campus-based request. Department FAO and/or Department Head is also required before submitting to the Facilities Management Department for review..
